Our Services
Post-Acute Care
We see patients admitted to skilled nursing facilities who are receiving rehabilitative therapy (physical, occupational, respiratory, and speech) in the hopes of getting them strong and medically stable enough to go home safely. These patients are seen up to 3 times per week. We facilitate our patients’ transitions of care back to their preexisting primary care providers.
Long-Term Care
We see patients who have transitioned to long-term care and are now living in a nursing facility. These patients are seen monthly for routine visits, or as often as needed should problems arise. Patients sometimes require and qualify for therapy services, or temporarily transition back to post-acute care should they require hospitalization.
Home Health
We see patients who have elected to receive various home health services through Mission Minded Home Health Care. We see these patients both in-person or via telemedicine with the assistance of a home health nurse. More comprehensive information on the services they provide is available on their website.
Hospice
Dr. Silva serves as hospice medical director for both Heart to Heart Hospice and Hospice Plus’s Dallas-area branches, supervising the care of patients who have elected to receive hospice care through these companies. Note
We do NOT have an outpatient clinic and so are ONLY able to care for patients at the [skilled] nursing facilities or through the home health + hospice services enumerated below. If this changes in the future, it will be reflected here.
