Our Services
Post-Acute Care
We see patients admitted to skilled nursing facilities who are receiving rehabilitative therapy (physical, occupational, respiratory, and speech) in the hopes of getting them strong and medically stable enough to go home safely.
Long-Term Care
We see patients who have transitioned to long-term care and are now living at a nursing facility. We see patients monthly for routine visits, or as often as needed should problems arise.
Note
We do not have an outpatient clinic and so are ONLY able to care for patients at the nursing / skilled nursing facilities listed below. If that changes in the future, it will be reflected here.
