Our Services
Post-Acute Care
We see patients admitted to skilled nursing facilities who are receiving rehabilitative therapy (physical, occupational, respiratory, and speech) in the hopes of getting them strong and medically stable enough to go home safely.
Long-Term Care
We see patients who have transitioned to long-term care and are now living at a nursing facility. We see patients monthly for routine visits, or as often as needed should problems arise.
Note
Unfortunately, we are NOT able to offer Primary Care services at this time. We do not have a clinic currently and are only able to care for patients admitted to our nursing / skilled nursing facilities. If that changes in the future, it will be reflected here.
